Organizations in Huron & Perth Counties have a way of making things happen!  They focus relentlessly on a Better Way and then through dialogue and hard work they make it happen.  Its been great working closely with all the Community Support Services agencies in Huron & Perth who together are creating a seamless, coordinated, easy way for people in their communities to get the services they need to stay at home.  What an awesome opportunity to facilitate this process.  Visionaries Kathy Scanlon and Jennifer Croft of One Care can be credited for dreaming of this Better Way and with partners, we have co-created the approach.  The model is so successful that we’ve won a South West Local Health Integration Quality Award as well as receiving the Minister’s Medal – Honour Roll.   Joseph Edward of InvOrg and his team have enabled the strategy to move forward by creating a platform that allows a ‘real time’ shared client record creating a much better client experience and significant efficiencies in the system.  We are now working at implementing the model across the South West LHIN region.  Exciting work ahead….
